About Us:
At Kids we’re dedicated to supporting families of young children with disabilities. Our accredited Portage early intervention service empowers children under five to reach their potential through structured play and family partnership. We value compassion, professionalism, and innovation, ensuring every child and family receives the highest standard of support.
Overall Purpose:
As a Portage Early Intervention Practitioner, you will deliver a home-based service that supports disabled children under five and their families. You will work closely with parents to implement structured, child-led play programs, monitor progress, and provide family guidance, all while contributing to a collaborative multidisciplinary network.
Key Responsibilities:
- Build strong, supportive partnerships with parents and carers.
- Conduct regular weekly home visits to families.
- Manage your own caseload efficiently and independently.
- Develop and implement tailored intervention plans, including weekly activities, progress reviews, and reports.
- Maintain accurate and timely records.
- Coordinate or attend Team Around the Child meetings.
- Participate in multidisciplinary assessments and interventions.
- Contribute to the wider Kids Portage network and project-specific initiatives.
- Adhere to legislation, policies, and procedures relevant to working with children.
- Engage in regular supervision and Continuing Professional Development (CPD).
Autonomy & Accountability:
- Navigate and manage a designated geographical area independently.
- Take full responsibility for direct engagement with children and families.
- Ensure all equipment and resources meet Health & Safety standards.
Essential Skills & Experience:
- In-depth knowledge of early childhood development and intervention strategies.
- Familiarity with developmental assessment tools for young children.
- Experience supporting parents of disabled children (birth to 5 years).
- Expertise in planning and structuring child-focused learning activities.
- Strong report writing and record-keeping skills.
- Proficiency in email and standard office software.
- Ability to maintain clear, legible notes for parents.
- Experience managing personal caseloads.
- Commitment to anti-discriminatory practices and safeguarding procedures.
